  In the United States the phrase "How are you" can entirely depend on the context of the moment. In a retail environment (at work) when directed towards customers the phrase is commonly synonomous with a friendly hello. This can be affected by where you place the emphasis in the sentence. Often if you are actually trying to elicit a response you can put the emphasis on "are" as in "How ARE you?" and even then the only expectation is a single word or short response. Rarely when dealing with strangers, or customers, are we really interested in the peaks and valleys of your everyday woes. We're simply respectfully acknowledging your existence.

  Of course this simple phrase can take more meaning depending on the level of intimacy we share with the other person. If you are a family member or close friend more of a response may be expected. How are you, coming from a confidant, priest or counselor may be asking you to take a more honest look yourself and find an answer even you did not anticipate.

  In general though, with the average stranger, how are you is just a slightly more courteous version of hello. I am generally pleased to recieve a "good, thanks, fine etc..." If you're really feeling vindictive the next time a person asks you "how are you" give them an earful, more than they really wanted. I use this with telemarketers, giving them a lot of fictitious VERY personal information. After all they asked, right?
